Our Case Management program provides goal-oriented, individualized support to help members become increasingly self-sufficient. We take a holistic approach—through assessment, planning, advocacy, coordination, and monitoring—to ensure that every member has the resources and guidance needed for long-term recovery and re-entry into the community.
How We Support You:
- Housing Placement – Assistance in securing safe and stable housing, including navigating housing applications, rental agreements, and transitional living opportunities.
- Work Placement – Job readiness training, résumé development, interview preparation, and connections to local employers to promote meaningful employment.
- I.D. Support – Guidance in obtaining essential documents like state identification, driver’s licenses, and birth certificates.
- Budget & Credit Management – Education on managing finances, creating budgets, improving credit, and building financial independence.
- Building Healthy Relationships – Support in strengthening family ties, improving communication, and fostering positive social connections.
- G.E.D. Assistance – Help enrolling in and completing GED programs, plus tutoring and test preparation resources.
- Nutrition & Self-Care – Education on healthy eating, fitness, and self-care routines to promote overall wellness.
- Computer, Phone & social media skills – Training to improve digital literacy, communication, and responsible use of technology for personal and professional growth.
- Legal Advocacy / Mitigation of Fines – Support with navigating the legal system, addressing fines, and advocating for fair resolutions.
- Retrieval Assistance (License / SSN) – Help recovering vital records and documents like Social Security cards and driver’s licenses that are often barriers to stability.
